Certificate Issuance (For Alumni and Former Students)

Ordering Certificates

*Due to the current renovation of the West Building, it may take longer than usual to issue certificates to those who fall under the following categories.
We appreciate your understanding.

Regular students (MC, DC): Those who completed their studies before 2001 (H13) (e.g., A0K0△△△△).
Non-regular students (Research students, etc.): Students who were enrolled in 2012 (H24) or earlier.

We accept requests for certificates at the counter, by mail, or by email. But please note that a self-addressed stamped envelope must be sent.
*We do not send certificates overseas. If you reside outside Japan, please designate a proxy living in Japan to receive certificates on your behalf.
In order to protect personal information, we ask applicants to show their IDs.
We apologize for the inconvenience and appreciate your cooperation.

Processing Times *Shipping time for mail and email

Processing Times: About 3 to 5 business days after application documents have been submitted.

*For applicants who fall under the following categories, it may take about 7 to 10 business days to issue certificates.

Regular students (MC, DC): Those who completed their studies before 2001 (H13) (e.g., A0K0△△△△).
Non-regular students (Research students, etc.): Students who were enrolled in 2012 (H24) or earlier.

In the case of email, the date of receipt will be the date the email is received, but after 5:00 p.m., the next day will be the date of receipt.
If you apply for certificates by mail or Email, please give yourself plenty of time to receive them. (We do not offer the option to select a date for delivery.)
Additional processing time may be required for ordering multiple certificates.

How to Apply In-person

Please complete the application form below and submit it to the Academic Affairs Office. Applications are accepted at the Academic Affairs Office during regular opening hours. Applicants are required to present a form of identification (e.g., student ID, driver’s license, national health insurance card, or residence card) at the counter. If a request is made through a proxy, the proxy is required to present their ID and submit the following documents: 1) Copy of the applicant’s ID, 2) Application Form, 3) Letter of Proxy(prescribed form). *Please note that some certificates cannot be issued on the same day.

How to Apply by Mail or Email

Please complete the application form and send it to the Academic Affairs Office with a copy of the applicant’s ID (e.g., student ID, driver’s license, national health insurance card, or residence card) and a self-addressed stamped envelope.
*The certificate application form and a copy of the applicant’s ID may be submitted by email.
However, in order to protect personal information, we recommend that you set a password when sending documents containing personal information as attachment files. Also please be careful in handling personal information, for example, inform us of the password in a separate email, and do not include personal information in the body of the email etc.

If you wish to receive the certificate by proxy, the following documents must be submitted.
1. Copy of the applicant’s ID (e.g., student ID, driver’s license, national health insurance card, or residence card)
2. Application form
3. Copy of the proxy’s ID (e.g., student ID, driver’s license, national health insurance card, or residence card)
4. Letter of Proxy
5. Self-addressed stamped envelope

*Submitted copies of IDs will be used solely for the certificate issuance.

Issuance fees are not required. Those who require confidential certificates in sealed envelopes, please refer to the higher price in the column for postage stamp cost above. Please indicate on the application form that you wish to receive express delivery, please add JPY 260 stamp. Those who wish to more than 10 items are advised to contact the Academic Affairs Office in advance. Please do NOT send cash instead of postage stamps.
